None of these ideas are intuitively obvious. They make sense only if one already has a theoretical orientation that finds it compelling and reasonable to think abstractly about populations, about individuals in numerators and denominators, about averages and distributions, about disease occurrence in space and time, and about disease as a definable entity apart from (as opposed to uniquely residing in) the individual persons in whom it is experienced—and hence diseased persons as countable cases.
